Endurance Psychology Pty Ltd t/a Wellness Experts (ABN 23 660 964 330) ("we", "us", "our") is committed to protecting your privacy. This policy explains how we collect, hold, use, and disclose personal information, including health information, in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the Health Records and Information Privacy Act 2002 (NSW).
1. The information we collect
We collect personal information necessary to provide our allied health and psychology services. This may include:
- Identifying details (name, date of birth, contact information)
- NDIS participant number, plan details and funding type
- Health and clinical information (diagnoses, history, goals, assessments, reports)
- Referrer details (GP, support coordinator, family member)
- Records of communications with us
2. How we collect it
We collect information directly from you whenever practicable — through our website referral form, by phone, email, in person, or through documents you provide. Where lawful and necessary, we may also collect information from your nominated GP, support coordinator, parent/guardian or another health provider.
3. Why we collect it
We use your information to:
- Assess suitability and respond to referrals
- Provide assessment, therapy and clinical care
- Coordinate with your other treating clinicians and supports (with your consent)
- Bill the NDIA, Medicare, plan managers, or you directly
- Comply with our legal, professional and regulatory obligations
- Improve our services in a de-identified way
4. Online referral form
Referrals submitted through this website are transmitted securely (over HTTPS) via a third-party form-handling service (Web3Forms) and delivered to admin@wellnessexperts.com.au. Web3Forms acts only as a transport service and does not use submission data for marketing. We ask that you do not include sensitive clinical details in the form — we will collect those securely once we have made contact.
5. Disclosure
We will not disclose your personal or health information to third parties without your consent, except where:
- It is required for your care and you have provided informed consent
- It is necessary to lessen or prevent a serious threat to life, health or safety
- It is required or authorised by law (e.g. mandatory reporting, subpoena, NDIS Commission)
- It is used to bill a funder you have nominated (NDIA, Medicare, plan manager)
We do not sell personal information and we do not use it for advertising.
6. Storage and security
Clinical records are stored in secure, access-controlled practice management systems hosted within Australia. Access is limited to authorised personnel on a need-to-know basis. We use reasonable technical and organisational safeguards — including encryption in transit, multi-factor authentication, and staff confidentiality obligations — to protect against misuse, loss, unauthorised access, modification or disclosure. Records are retained for the minimum periods required by law (generally 7 years for adults, or until age 25 for minors) and securely destroyed thereafter.
7. Cookies and analytics
Our website does not use third-party advertising trackers. Only essential cookies required to operate the site may be set. If we add analytics in future, we will update this policy and provide a clear opt-in.
8. Your rights
You have the right to:
- Request access to the personal and health information we hold about you
- Request correction of inaccurate or out-of-date information
- Withdraw consent for non-essential uses of your information
- Lodge a complaint about how we have handled your information
